Important Patient Information

Please activate your patient portal at UTPhysicians.com/myuthealthhouston

  • We ask that you arrive at least 15 minutes prior to your appointment. If you are running late or cannot make your appointment, please call our office as soon as possible.
  • If you are more than 15 minutes late, your physician will determine if you can be worked into the schedule or if rescheduling is necessary.
  • Please schedule your follow-up appointment when checking out after today’s office visit. Physician’s schedules fill quickly so waiting to schedule may delay your follow-up. If you are unable to schedule an appointment at check out, please call us as soon as possible to schedule.
  • Please allow at least 24-48 hours for a return phone call when messages are left with clinic staff.
  • Please bring a copy of your most current medication list for review and reconciliation. Please report any changes or updates in your patient portal.
  • If refills are needed, be sure to notify your physician or nursing staff before leaving the office. After your visit, please request refills through your pharmacy. You may also send a request to the clinic via your patient portal. Please be sure the correct medication, strength, and dose are on your request.
  • Allow 7-10 business days for prescriptions that require prior authorization. Your pharmacy on file will contact our office for a prior authorization request via fax. Some prior authorizations may take longer based on the medication and the potential need for patient assistance programs.
  • For controlled medications, we ask that you allow at least 48-72 hours for the refill to be authorized by your physician.
  • If your physician orders additional testing or referrals such as physical therapy, occupational therapy, home health, radiology exams/tests, neuro psych, etc., you will be contacted directly by the clinic or facility to which you were referred to as our office does not schedule referral appointments.
  • If you have not been contacted in at least 5-7 business days, please contact the facility that you were referred to first. Please see the facility directory below.
  • Many of your results may be viewed online via the MyUTHealth Houston patient portal.
  • Please allow at least 7-14 days for notification of your results. Some providers may call you to discuss results or send you a message via the patient portal.
  • If you need any of your UT Physicians medical records to be sent to another provider or clinic, please contact Medical Records at 832-325-6543. You may also submit a request online at UTPhysicians.com/medical-records.
  • Please sign up for the MyUTHealth Houston patient portal. Your telemedicine visits are accessed via your patient portal. It is a very helpful and secure way to communicate with your physician and/or clinical team. It is also an easier and quicker way to address any concerns or questions that you may have. You can sign up when checking in for your appointment at the front desk or by visiting UTPhysicians.com/myuthealthhouston..
