Care Everywhere

When visiting a health care facility outside of the UT Physicians and UTHealth Houston network, the biggest benefit is that Care Everywhere gives health care providers secure access to your medical information to help them make important medical decisions regarding your care. Exchanging health information between medical organizations is traditionally a time-consuming process. With Care Everywhere, your health information can be transmitted electronically in a matter of seconds. Your UT Physicians and UTHealth Houston providers will also have the most updated medical information so we can better meet your health care needs during your next scheduled visit with us.

Your health care providers at UT Physicians, UTHealth Houston, and affiliate care providers that you have a relationship with can view your medical information.

Patient health information includes information on your personal electronic health record. Shared information may include allergies, medications, medical history, immunizations, patient visit summaries, test results, and diagnoses.

If you are a UT Physicians patient, there is no sign-up process for Care Everywhere. You are automatically enrolled. If you are a patient at HCPC, you will have the opportunity to sign up for Care Everywhere upon discharge.

Yes. You or your legally authorized representative may opt out of Care Everywhere at any time by notifying UTHealth Houston / UT Physicians in writing.

Houston, Texas 77030 You can also opt-out by logging into the MyUTHealth Houston patient portal and accessing Authorize Sharing under the main menu. Once on that page, you can click on send us a customer service request and provide a communication that you wish to opt-out.

At this time, we can only transfer your personal health record electronically to and from other organizations that use a compatible electronic health record software.

No, there is no fee. Care Everywhere is a free service provided to help improve the quality of care that you are receiving here at UT Physicians, UTHealth Houston, and other health care organizations.
