What is MyUTHealth Houston?
MyUTHealth Houston offers patients personalized and secure online access to their medical records. Patients are able to securely use the internet to help manage and receive information about their health. With MyUTHealth Houston, which is part of the MyChart medical app, you can:

- View your health summary from the MyUTHealth Houston electronic health record

Now Available: MyChart Central
MyChart Central, a new Epic tool, allows patients to access all their MyChart accounts from different health care organizations in one secure place. With just one login, patients can check their health records, appointments, and messages from different hospitals or clinics.
This platform is useful for patients with more than one health care provider. Patients can also use MyChart Central through the MyChart app on their phone, so it’s easy to stay connected to doctors anytime, anywhere.
At UTHealth Houston, MyChart is known as MyUTHealth Houston, but both names refer to the same patient portal.
MyChart Central helps you keep all your MyChart accounts in one place. If you receive care from more than one health care organization, MyChart Central lets you link your accounts so you can use a single login to access them all. You can also access MyChart Central in the MyUTHealth Houston mobile application.
When you create a MyChart Central account, you also create an Epic ID—an email address and password combination that you use to log in to MyChart Central. Your Epic ID can also log you in to your MyUTHealth Houston account or any other health care organization that you’ve linked to MyChart Central.
You need an existing MyUTHealth Houston account to create a MyChart Central account.
- Visit MyUTHealthHouston.org or use your MyUTHealth Houston mobile app and log in with your credentials.
- From within your MyUTHealth Houston account, go to Link My Accounts from the main menu and select Get Started under One Login for Your MyChart Accounts.

MyUTHealth Houston lets you continue communicating with your doctor and managing your health record. However, when you log in through MyChart Central (using your Epic ID), you can access all your linked health care organizations — including MyUTHealth Houston — from one convenient location.

MyUTHealth Houston offers a free service designed to support your health care needs. Sometimes, your questions might require scheduling an appointment to ensure the best care. We offer flexible appointment options, including in-person visits, telehealth sessions, and E-Visits. For more details, please review our policies.
Patients who wish to participate have multiple ways to sign up. Patients are issued a MyUTHealth Houston access code during their clinic visit. Patients can be texted or emailed a code during their clinic visit by UTHealth Houston staff. This code will enable you to login and create your own user ID and password. If you were not issued an access code, you can also sign up online by clicking on “Sign up now”, then clicking on “Sign up online” under “No Activation Code,” you can then match yourself with our records or verify identity with a third party. After successful verification, you can create your own username and password.

After I Have Enrolled
Several different methods of MyUTHealth Houston signup might be used by different departments:
- Clinic staff might sign you up directly while you’re at the front desk or in the exam room.
- You might receive a MyUTHealth Houston activation code on your After Visit Summary or on a billing statement.
- You might receive a text message or email with an activation code when you come in for a visit.
- You might be able to use self-signup online to create a MyUTHealth Houston account by matching your information against what is on file in your medical record or with third-party identity verification.
If you don’t have an activation code, you can request one on the sign-up page. You can also contact us at [email protected]. After we verify your information, you will receive a new code.
For your security, your activation code expires after a set period of time and is no longer valid after the first time you use it. If you still have problems, email us at [email protected] or call our MyUTHealth Houston Patient Support Line at 855-316-4256.
Your Medical Record
Your test results will be released to your MyUTHealth Houston account as soon as they are available. If you have proxy access to a family member’s MyUTHealth Houston account, access to test results may vary.
Your provider is able to determine which types of test results are able to be accessed through MyUTHealth Houston. Further, tests of a very sensitive nature are not released to MyUTHealth Houston.
Your MyUTHealth Houston information comes directly from your electronic medical record at your doctor’s office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.
You will generally receive an answer within 1-2 business days. Please note that MyUTHealth Houston should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.
MyUTHealth Houston can send reminders for preventive care, test results, statements, payment reminders, and more. Patients may adjust their notification settings. UTHealth Houston does not have an integrated pharmacy that dispenses medications, so refill due messages will not be available.
MyUTHealth Houston for My Family
Yes, you can. This is called proxy access and a parent, guardian, spouse, or other authorized family member logs in to their own MyUTHealth Houston account and connects to information regarding their family member.
If the patient is over 18, they should complete a Proxy Consent Form and return it to one of our clinics to request access to this service or have the patient click “Share My Record” from the “Health” menu. From there, the patient can select “Friends and family access” and modify or add new access to their health record.
The availability of information in the family member’s account varies based on the patient age and access granted.
Adults can grant varying levels of proxy access to other adults or caregivers. Please discuss these options with your clinic and have the patient sign the Proxy Consent form the next time they are in clinic.
Yes, you will be notified of changes to your proxy access upon logging into MyUTHealth Houston.
MyUTHealth Houston offers direct access to your personal health record and communicating about another individual’s information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.
No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyUTHealth Houston account.
Technical Questions
We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional email, all MyUTHealth Houston messaging is done while you are securely logged on to our website or mobile app.
MyUTHealth Houston is owned and operated by UTHealth Houston and is fully compliant with federal and state laws pertaining to your privacy. Your name and email address will be treated with the same care and privacy given your health records and will never be sold or leased by MyUTHealth Houston. At UTHealth Houston, our pledge is to protect your privacy. While logged into MyUTHealth Houston, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of the patient portal. We recommend that you log out of your MyUTHealth Houston session if you need to leave your computer for even a short period of time.
You will need either a personal email or personal mobile phone number and access to a computer, mobile device or tablet connected to the Internet. For a mobile device or tablet, you will be able to download MyUTHealth Houston from Google Play or Apple’s app store.

It is a method of confirming a user’s claimed identity by using a combination of two different steps. This would be your password and a verification code sent to you via email or text message.
To provide additional security during MyUTHealth Houston signup, login, password reset, or password expiration. Two-step verification minimizes identity theft and helps prevent unauthorized disclosure of your electronic health information.
When two-step verification is used after a patient enters their MyUTHealth Houston username and password, the user is taken to the additional security verification page to enter a verification code sent to them via email or text messaging.

You can opt out of two-step verification by clicking “turn off two-step verification” under security settings in the profile menu at the top of the MyUTHealth Houston screen. However, we recommend keeping it on to help protect your privacy.
